Perimenopause can feel like your body’s staging a protest—and you’re the one stuck holding the megaphone. Between the brain fog, mood swings, night sweats, and hair loss (why there, Mother Nature?), you might be wondering:

You’ve probably heard whispers about bioidentical hormones. Maybe your best friend swears by her estrogen cream. Maybe you’re using pellets or a compounded cream and wondering, Is this stuff really working—or is it just expensive lotion?
Let’s break it all down—with no fluff, no fear, and a healthy dose of sass.
Think of bioidentical hormones as custom knockoffs of the ones your body naturally makes. They’re usually derived from plants (like soy or yams), then made in a lab to be molecularly identical to your own estrogen, progesterone, or testosterone.
They can come in:
Creams or gels (rubbed on like fancy moisturizer)
Pills or lozenges
Vaginal inserts (hello, hydration!)
Pellets (inserted under your skin and released slowly)
Here’s where things get juicy.
Some studies—and plenty of respected docs—say yes, especially when it comes to transdermal estrogen and micronized progesterone. Bioidentical versions may carry lower risks for blood clots and breast cancer than the synthetic stuff used in older hormone replacement therapies (HRT).
Some experts suggest that using bioidentical hormones through the skin (like patches or creams) could be a safer option than older, synthetic hormone therapies.
BUT—and it’s a big but—the bioidentical hormone world isn’t all regulated the same. Some versions are FDA-approved. Others? Compounded by a local pharmacy with no standardized oversight. That’s where things get murky.
Real talk? I was on testosterone pellets for over two years. At first, they helped—hello, energy and mood boost! But then came the hair thinning at my temples (ugh), and I was prescribed Finasteride to manage that side effect.
I’ve since switched to a compounded testosterone cream, and if I’m being honest, I wouldn’t go the pellet route again. It felt like too much hormone all at once, and adjusting the dose? Not exactly easy when it’s literally under your skin.

Controversy: Some docs love them. Others don’t. You may get conflicting advice
Because many compounded bioidentical hormones aren’t regulated by the FDA, there’s concern about inconsistent dosing and varying quality between pharmacies.
Remember the 2002 Women’s Health Initiative (WHI) study that scared everyone off hormones? Here’s the truth: it focused on synthetic hormones given to older postmenopausal women. Not perimenopausal women. Not bioidenticals.
Many experts now believe the study caused more panic than prevention.
The WHI study caused widespread fear about hormone therapy, but many now agree it didn’t account for the different needs of younger, perimenopausal women or distinguish between types of hormones.
Bottom line? We’ve learned a lot since then—and BHRT deserves a seat at the table.
There’s no gold-star answer here. Some women swear by BHRT. Others feel better with lifestyle changes, nutrition tweaks, or targeted supplements. Some go the traditional HRT route and feel great.
You do not need to be suffering in silence—but you do need a hormone-literate provider who will actually listen, test your levels, and not rush out the door while you’re still talking.
Thriving in perimenopause means tuning into your body, trusting what it’s telling you, and working with a provider who respects your unique needs.
