Let’s Talk Hormones (Like, Really Talk)

Perimenopause can feel like your body is staging a full-blown protest, and you’re the one stuck holding the megaphone. Between brain fog, mood swings, night sweats, and hair loss (why that spot, Mother Nature?), it’s very reasonable to start wondering:

Is it time to bring in hormonal reinforcements?

You’ve probably heard whispers about hormone therapy. Maybe bioidentical hormones. Maybe patches, pills, pellets, creams, or vaginal estrogen. Maybe your best friend swears by her estrogen patch, while your doctor barely mentions hormones at all.

And if you’re already using something, you might be asking yourself, “Is this actually helping, or is it just expensive lotion with a side of hope?”

Let’s break it down. No fear. No fluff. No one-size-fits-all answers.


🧬 What Do we Actually Mean by Hormone Therapy?

Hormone therapy is a broad term. It simply means using estrogen, progesterone, and sometimes testosterone to support the body when your own hormone production becomes inconsistent or declines.

That support can come in different forms:

  • Estrogen, progesterone, or testosterone

  • Delivered as patches, creams, gels, pills, vaginal inserts, or pellets

  • Either FDA-approved or compounded

  • Bioidentical or synthetic

This is where things often get confusing, because the conversation tends to focus on “bioidentical versus synthetic,” when the real question should be:

What does your body need, and how can it be supported safely and effectively?


🔍 Are Bioidentical Hormones Safer Than Synthetic Ones?

This is where nuance matters.

Some research, and many menopause-literate clinicians, suggest that certain forms of hormone therapy, particularly transdermal estrogen and micronized progesterone, may carry lower risks than older synthetic formulations used decades ago.

That said, not all bioidentical hormone therapy is the same.

FDA-approved options are regulated, standardized, and studied. Compounded hormones can be helpful in specific cases, but they vary in quality and consistency depending on the pharmacy and provider.

So it’s not about “bioidentical good, synthetic bad.”
It’s about the right hormone, the right dose, the right delivery method, and the right timing.


🩸 My Experience: Pellets, Testosterone & Hair Drama

Real talk.

I used testosterone pellets for over two years. At first, they helped. Energy improved. Mood lifted. I felt more like myself again.

Then came the hair thinning at my temples. Not subtle. Not fun.

I was prescribed Finasteride to manage that side effect, and eventually I decided to switch from pellets to a compounded testosterone cream.

Looking back, pellets felt like too much hormone all at once. Once they’re in, they’re in. Adjusting the dose isn’t exactly simple when the hormone is literally under your skin.

Would pellets be right for some women? Possibly.
Would I choose them again? No.

And that’s exactly why I encourage women to ask more questions before starting any form of hormone therapy, especially ones that are harder to adjust.

The Potential Benefits of Hormone Therapy

For many women, hormone therapy can be life-changing.

Potential benefits may include:

  • Reduced hot flashes and night sweats

  • Improved sleep and energy

  • Better mood stability

  • Reduced anxiety

  • Improved vaginal and urinary health

  • Better quality of life overall

This does not mean hormones are a magic fix. But for women struggling through perimenopause, they can be a powerful tool when used thoughtfully.


⚠️ The Not-So-Glowy Parts (because we're being honest)

Hormone therapy can be incredibly helpful, but it isn’t all sunshine and instant relief. There are real considerations that deserve to be talked about openly.

Here are a few of the less glamorous parts.

Cost:
Custom compounded hormones are often not covered by insurance, which can make them expensive over time.

Consistency:
Compounded hormones can vary more than FDA-approved options. That doesn’t mean they’re bad, but it does mean you need a skilled provider and a high-quality compounding pharmacy you trust.

Controversy:
Some doctors are very supportive of bioidentical or compounded hormone therapy. Others are not. You may hear conflicting opinions, and that can be confusing and frustrating.

Because many compounded hormones are not FDA regulated, there are valid concerns about dosing consistency and quality control between pharmacies. This is why provider experience and oversight matter so much.

This isn’t about fear. It’s about informed choice.


🧪 The Word About the WHI Study

The Women’s Health Initiative study in the early 2000s scared an entire generation of women away from hormone therapy.

What is often left out is that the study focused on older postmenopausal women using specific synthetic hormones. It did not reflect today’s understanding of timing, dosage, delivery methods, or the needs of perimenopausal women.

Since then, research and clinical practice have evolved significantly. Hormone therapy today is not the same as it was twenty years ago.


💡 Is Hormone Therapy Right for You?

There is no universal answer.

Some women feel incredible with hormone therapy.
Some feel better focusing on lifestyle, nutrition, and nervous system support first.
Some use a combination of both.

What matters most is that you are informed, supported, and not dismissed.

You do not need to suffer in silence. You do need a provider who will listen, test appropriately, explain your options, and treat you like a whole human being.

Hormone therapy is not about chasing youth or fixing something that is “wrong” with you.

It is about support.

Whether you choose FDA-approved hormone therapy, bioidenticals, compounded options, or none at all, you deserve real information and real choice.
