Heavy bleeding during perimenopause isn’t just inconvenient—it can be life-altering. I still remember the moment I realized my periods weren’t just heavy; they were uncontrollable. An overflowing Diva Cup, stained sheets, and the constant fear of leaks became my reality. If you’re struggling with extreme bleeding, know that you’re not alone—and there are real solutions that can help.
In my late 30s, my cycles were unpredictable, but I chalked it up to stress, diet, or just bad luck. A few irregular periods didn’t seem like a big deal. But by my mid-40s, things escalated. My period wasn’t just inconsistent; it was overwhelming. The blood loss became so intense that I dreaded leaving the house. My Diva Cup would overflow within hours, soaking through layers of protection. More nights than I care to count, I woke up drenched in blood, feeling like I had lost control of my own body.
I kept extra clothes soaking in the sink, lived in fear of leaks, and endured an ever-present ache in my vagina from constantly wearing my Diva Cup. The worst part? There was rarely a true break. If I wasn’t actively bleeding, I was spotting. It felt endless.
Anemia crept in like an unwelcome guest, draining me of energy and making me feel perpetually exhausted. I started iron supplements, but looking back, I should have tried iron infusions. If you’re in a similar boat, know this: iron infusions can be a game changer (always check with your doctor).
Perimenopause isn’t just about hot flashes and mood swings—it can be a rollercoaster of hormonal chaos that throws your body into survival mode. One of the most distressing symptoms? Heavy, prolonged bleeding.

1. Hormonal Havoc
During perimenopause, estrogen and progesterone become erratic. Estrogen surges can cause the uterine lining to build up excessively. When your body finally sheds it? It’s a bloodbath.
2. Anovulatory Cycles (Your Body Forgetting to Ovulate)
As ovulation becomes irregular, progesterone—the hormone that keeps estrogen in check—drops. Without progesterone, estrogen takes over, thickening the uterine lining until the inevitable heavy bleed.
3. Uterine Fibroids & Polyps
These benign growths can worsen bleeding. If your periods suddenly become extreme, it’s worth checking for fibroids or polyps with your doctor.
When I finally saw a gynecologist specializing in hormones and functional medicine, I realized I had more options than just waiting it out. And after trying different things, I found what truly worked for me.
💙 Mirena IUD: The Game Changer I Chose
After lots of research and discussion with my doctor, I decided on the Mirena IUD—and honestly, it was a lifesaver. It releases a small dose of progesterone directly into the uterus, thinning the lining and reducing bleeding over time. Within months, my periods became lighter—sometimes non-existent—and the constant fear of flooding disappeared.
💙 Oral Progesterone: Bringing Balance
In addition to the IUD, I also used bioidentical progesterone pills. I still do. They helped balance out the estrogen surges that were making my bleeding worse. During perimenopause, it’s common for progesterone to drop while estrogen spikes unpredictably—this mismatch can lead to a thicker uterine lining and heavier periods. The added progesterone brought more stability to my cycle and eased the intensity of my bleeding.
💙 Endometrial Ablation: A Drastic (But Effective) Option
I didn’t end up choosing this route, but I learned about it while exploring my options. For those who don’t respond to other treatments, endometrial ablation (removing the uterine lining) can reduce or even eliminate periods. It’s worth discussing with your doctor if you’re struggling with extreme bleeding.
While medical treatments helped regulate my cycle, nutrition and supplements were my secret weapons for managing energy levels and overall health.
Iron Power: Fighting Anemia
Anemia made me feel like a zombie. I loaded up on iron-rich foods: lean red meat, spinach, lentils, and pumpkin seeds. I also paired iron with vitamin C for better absorption (think steak and bell peppers or spinach and citrus dressing).
Anti-Inflammatory Diet: Keeping Hormones in Check
I ditched processed foods, cut back on sugar and eliminated alcohol, and prioritized:
• Fatty fish (omega-3s help balance hormones)
• Nuts & seeds (healthy fats + magnesium for muscle relaxation)
• Leafy greens (nutrients + fiber for gut health)
Iron Supplements: Crucial for replenishing lost iron. (Always take with vitamin C.)
Vitamin D: Supports mood and bone health.
Magnesium: A game changer for sleep, stress, and muscle relaxation.
B Complex: Essential for energy and brain health.
Omega-3s: Help reduce inflammation and support heart health.
You don’t have to suffer in silence. If your doctor dismisses your concerns, find one who listens.
Heavy bleeding isn’t just an inconvenience—it can be dangerous. If you’re feeling drained, ask about iron infusions.
Hormonal support isn’t one-size-fits-all. IUDs and oral progesterone may offer relief, while other options like ablation are permanent and require careful discussion with a trusted doctor.
Diet and lifestyle matter. Supporting your body with nutrient-dense foods and supplements can make all the difference.
Heavy bleeding is one of the most exhausting and frustrating symptoms of perimenopause, but you don’t have to let it control your life. With the right medical guidance, lifestyle changes, and support, you can take back your power.
